What Are Custom Orthotics?
Custom orthotics are prescription medical devices worn inside your shoes. Unlike over-the-counter insoles, custom orthotics are specifically designed for your unique foot shape, biomechanics, and condition. They correct abnormal foot motion, redistribute pressure, and provide support where you need it most.
Think of orthotics like prescription eyeglasses for your feet — they correct the way your feet function, not just cushion the bottom.
Who Can Benefit?
- Plantar Fasciitis (heel pain)
- Flat Feet (overpronation)
- High Arches (underpronation/supination)
- Achilles Tendinitis
- Posterior Tibial Tendinitis
- Bunions
- Hammertoes
- Morton's Neuroma
- Arthritis (foot or ankle)
- Diabetic foot conditions
How Do They Work?
Custom orthotics work by controlling abnormal foot motion and realigning the foot and ankle into a more efficient position. This reduces stress on painful structures, improves gait efficiency, and prevents reinjury.
Types of Orthotics
- Rigid orthotics – Made from carbon fiber or hard plastic. Control motion in the foot and ankle. Best for flat feet, overpronation, and arthritis.
- Semi-rigid orthotics – Provide a balance of support and cushion. Often used for athletic activities.
- Accommodative orthotics – Softer devices that cushion and protect sensitive areas. Ideal for diabetics, arthritic feet, or patients with pressure points.
The Fitting Process at Pathways Foot & Ankle
- Step 1 – Comprehensive Evaluation – Dr. Clifford examines your feet, watches you walk (gait analysis), and assesses your range of motion.
- Step 2 – Foam Impression Casting – We use high-quality foam impression molds to capture the exact 3D shape of your feet.
- Step 3 – Prescription Design – Based on your diagnosis, Dr. Clifford specifies the materials, density, arch height, and corrective features needed.
- Step 4 – Fabrication – Your custom orthotics are manufactured in a specialized lab (typically 1-2 weeks).
- Step 5 – Fitting and Adjustment – You return to try on your orthotics. Dr. Clifford makes any necessary adjustments and teaches you how to use them.
Custom vs. Over-the-Counter (OTC)

| Lifespan | 3-5 years with normal use | 3-6 months |
Getting Used to Your Orthotics
- Break-in period – Wear them for 1-2 hours on the first day, then gradually increase over 1-2 weeks.
- Expect some adjustment – Mild soreness or a "different" feeling is normal as your feet adapt.
- Start with low-impact activities – Walking, then progress to more demanding activities.
- Shoe compatibility – Orthotics work best in shoes with removable insoles and a wide toe box.
- Follow-up – Dr. Clifford will schedule a follow-up to ensure proper fit and function.
Maintenance Tips
- Wipe clean with a damp cloth – do not submerge in water
- Replace when worn (typically every 3-5 years)
- Use in multiple pairs of shoes – order additional pairs if needed
